Medicaid Service Coordination (MSC) assists individuals with developmental
disabilities to achieve their dreams and aspirations. Individualization is the
MSC watchword and the foundation of all services delivered to our consumers.

MSC begins with Person-Centered Planning that builds on the capabilities
and strengths of the consumer to enable him or her to plan a feasible and
desirable future and life-style.

Promoting choice, community participation and encouraging the consumer’s
input at every step of the planning process are integral elements of our MSC
program. Our MSC consumers are actively involved in their own program and
plans right from the start!

A key feature of this program is the circle of support that every MSC consumer has: family, friends, a special teacher, or a clergy-member are all possible “circle of support” members. They provide the support, practical help and encouragement that make all the difference to consumers learning to be more independent and to handle the challenges of community life.

The MSC Coordinator works with each individual to develop an Individualized Service Plan (ISP), the road map to activities and services of special interest to the individual consumer, to learning independent skills and to achieving the satisfaction of being an active and valued community member.

The MSC Coordinator receives competency training in core services as well as ongoing staff development training to maintain the quality of services delivered, to help the consumers protect their rights and safety, and to ensure programmatic accountability.
